How Much Should a Med Spa Owner Pay Themselves?

A practical way to set owner pay that rewards your work without starving the business of cash.

Many med spa owners pay themselves whatever is left at the end of the month, or nothing at all during growth. Both make it harder to see whether the business really works. A simple policy brings clarity.

Separate two things

1. Pay for the work you do

If you work as a provider, medical director or manager, that role has a market value. Paying yourself something close to it, through payroll or a consistent draw, shows the true cost of running the clinic. If you stopped working in the business, you would have to pay someone else.

2. Return on owning the business

What remains after every cost, including fair pay for your role, is profit. Distributions from that profit are your return as an owner.

Why it matters

If you pay yourself nothing for your role, the clinic looks more profitable than it would be without you. That matters when you think about hiring a replacement provider, selling the business or taking on a partner.

A simple policy

  1. Set your role pay at a sustainable level based on a market rate and the clinic's current size.
  2. Set aside taxes from profit in a separate account each month.
  3. Keep a reserve of several weeks of operating costs before taking distributions.
  4. Distribute a share of remaining profit monthly or quarterly, based on actual cash available after reserves and planned investments.

Watch for these pitfalls

  • Taking distributions from cash that belongs to prepaid packages and memberships
  • Drawing based on revenue rather than profit
  • Skipping tax set-asides and facing a large bill later
  • Paying yourself irregularly, which makes monthly results hard to compare

Talk to your CPA about structure

How owner pay is structured, as salary, draws or distributions, has tax consequences that depend on your entity type. Agree on the structure with your CPA, then apply the policy consistently.
