For most med spas, payroll is the single largest cost. It also grows quietly: a raise here, an extra front-desk shift there, a new provider hired ahead of demand. Tracking it as a percentage of revenue every month keeps that growth visible.
What to include
- Wages and salaries
- Commission and bonuses
- Employer payroll taxes
- Benefits and retirement contributions
- Contractor payments for providers or staff
Split it in two
Provider payroll
Pay for the people who perform treatments. It should move broadly with revenue, especially under commission plans.
Support payroll
Front desk, coordinators, managers and admin. This is mostly fixed. As revenue grows, support payroll as a percentage should fall. If it rises, staffing is growing faster than the business.
Read the trend, not one month
Payroll percentage moves with seasonality. A slow month makes it look high even if staffing is right. Compare each month with the same month last year and look at a rolling three-month average.
Warning signs
- Support payroll percentage rising for three months in a row
- Provider payroll rising faster than provider revenue
- Overtime becoming routine
- New hires whose schedules are less than half booked after their ramp-up period
Owner pay
If the owner works as a provider or manager and is paid through distributions instead of payroll, the payroll percentage understates the true cost of running the business. Include a market-rate salary for the owner's role when comparing.
What to do when it is too high
- Check utilization. Are providers fully booked? Filling schedules lowers the percentage without cutting anyone.
- Review support staffing against hours with the most check-ins.
- Review pricing. If prices lag costs, payroll looks high because revenue is too low.
- Review compensation plans for alignment with margin.
Payroll is an investment in capacity and client experience. The goal is not the lowest number, but a number that grows more slowly than revenue.
