When a clinic is quiet, the goal is simply more bookings. When it is busy, the constraint changes: you have a fixed number of rooms and provider hours, and the question becomes which services make the best use of them. That is what profit per room hour measures.
The calculation
For each service:
- Find contribution margin per treatment: net price minus product cost minus provider pay.
- Find the total room time it uses, including setup, cleaning and charting.
- Divide contribution margin by room hours.
For example, a 30-minute neuromodulator visit contributing $170 earns $340 per room hour. A 75-minute facial contributing $150 earns $120 per room hour. A 45-minute laser session contributing $260 earns about $347 per room hour.
Why it changes decisions
Scheduling
Prime slots, such as evenings and Saturdays, should go to the services with the highest contribution per hour, as long as clients still get what they need. Long, low-margin services can move to quieter times.
Pricing
If a popular service has low contribution per hour and your rooms are full, that is a strong signal its price is too low or its time is too long.
Capacity planning
Total available room hours multiplied by your average contribution per hour gives the ceiling on what the clinic can earn at current prices. If that ceiling is close to your costs, you need higher-value services, better pricing or more capacity.
Track utilization alongside it
Profit per room hour only matters for hours that are booked. Track room and provider utilization, meaning booked hours divided by available hours, every week. A clinic at 60% utilization has a demand problem; one at 90% has a capacity and pricing opportunity.
Watch no-shows and late cancellations
An empty slot earns nothing. Multiply no-show hours by your average contribution per hour to see what no-shows really cost each month. That number usually justifies a deposit or cancellation policy.
